Have Your Say on Plans to Join Up Health Teams in Dorset

People suffering with long term illnesses and their carers are needed to help develop a new partnership in Dorset.
Plans are being discussed to join up community health and social care teams under what's being called the "Better Together" programme.
Leaders say it the move will help people who're vulnerable and most at risk and they're now inviting feedback via e-mail, in the post or via an online survey.
Cllr Spencer Flower, Leader at Dorset County Council, said "Many excellent projects are being developed by working together. Staff from all agencies can readily identify which people are most at risk, such as those going into hospital. It will be possible to put together combined packages of care coupled with support and lifestyle advice to keep them healthier and independent for much longer".
Feedback on how people's views inform the final model of care will be provided later in the year.
The Better Together programme is a Dorset-wide initiative that will radically change the way health and adult social care services are delivered in the future.
