Respiratory Nurse Specialist

Two Harbours Healthcare Ltd is a GP Federation based in the seaside town of Weymouth on the beautiful Jurassic Coast in Dorset. It represents the six Weymouth & Portland practices and also works in collaboration with the local community trust and secondary care. We have an exciting opportunity to recruit a band 7 Respiratory Specialist Nurse to join the Community Respiratory Team within the Weymouth & Portland Locality. This will be a collaborative role working closely with GP Practices, the Local Trust and Secondary Care. The post would suit an experienced Respiratory Nurse Specialist or an individual who wishes to further specialise in the exciting, expanding and challenging field of Respiratory. The successful applicant will work as part of a multidisciplinary respiratory network in the Locality, leading the Community respiratory team, as well as providing education to patients and other health care professionals. The successful applicant will need to have some experience in the field of respiratory, be confident teaching groups of either patients or other health professionals, have excellent interpersonal skills and be innovative, enthusiastic and happy working autonomously. To plan and provide the coordination of care for patients with respiratory disease in the Weymouth and Portland Locality. To provide a high standard of evidence-based care for respiratory patients, carers and other health professionals, contributing to the effective management of both acute and chronic phases of respiratory problems in the community. This will include:

  • Providing local leadership to the community respiratory team
  • Attending and leading monthly community respiratory MDT meetings
  • Providing sessions of asthma and COPD reviews for complex patients – possibly in an MDT style ‘carousel’ clinic
  • Providing (in conjunction with DAIRS) advice and support for primary care and community staff
  • Liaising with other teams (eg palliative care, social care)
  • Supporting practice nurses with patient reviews and spirometry skills (annual review)
  • Providing support to patients at home during exacerbations
  • Providing follow up of patients after admission
  • Ensuring optimum uptake for Pulmonary Rehabilitation
  • Supporting the development of case finding processes
  • Providing education to locality health care professionals
  • Working towards a Locality Exhaled nitric oxide service
  • Providing assessment of patients for nebulisers
  • Identifying patients for home oxygen therapy and liaising with home oxygen service


To lead the delivery of community service provision for patients with acute/chronic respiratory diseases. The post holder will work to develop seamless care for this patient group between primary care, integrated community care and secondary care.

Provide leadership and ensure high standards of nursing care are maintained in the management of patients with respiratory disease.

Provide specialist advice as requested/required across the locality.


This post forms part of a newly developed Locality Respiratory Team including a GP Respiratory, a Locality-linked Respiratory Consultant, a Locality Respiratory Band 6 and MDT coordinator.

Overall governance is by Two Harbours Federation Board, with line management by the Federation Secretary.

Team governance will be provided by the Respiratory ICPCS governance board which will meet at least every 2 months.

Clinical Supervision is through the GP lead and Respiratory Consultant, with peer support from the Dorset Adult Integrated Respiratory Service based at DCH.


The post holder will be expected to communicate with staff at all levels throughout the organisation and external agencies as well as patients, carers, visitors and the general public. Of particular importance is the development and maintenance of effective communication and working relationships with, and between, other members of the Locality Respiratory Team, the Locality Integrated Care Team, Weymouth GP Practices and DAIRS.


Responsibility for Patients
  • Ability to communicate effectively at all levels
  • Specific clinical knowledge in; Chronic disease management, smoking cessation, COPD and other respiratory conditions, nursing assessments, nursing in the home, care planning / review and health and safety procedures.
  • To maintain accurate documentation and ensure timely delivery of information to relevant parties regarding any significant deviation in the patients’ health status and/or management.
  • To manage own caseload of patients and ensure treatment optimisation through agreed protocols. To optimise the patients’ overall management, quality of life and health outcomes.
  • To provide concurrent education, information and advice to patients and their families, regarding pharmacological and non-pharmacological management. 



Responsibility for Policy and Service Development
  • The post holder will be required to contribute to the development of service policies.
  • Maintain a database of respiratory patients.
Responsibility for Financial and Physical Resources
  • Personal duty of care in relation to equipment and resources.
Responsibility for Staff
  • To provide leadership to Locality Respiratory Team
  • They will be expected to supervise, manage and/or teach staff or students.
  • Liaison with and some training of Weymouth practice clinical staff
  • To provide mentoring and supervision for colleagues.
  • Provide training in own discipline.
Responsibility for Information Resources
  • It is anticipated that the post holder will record clinical data; main clinical record using System 1
  • To work with MDT coordinator to establish and maintain systems and structures for clinical governance.
