Police Constable David Williams, of Dorchester police, said: "I understand that Matthew's family are becoming increasingly concerned because they haven't seen him for almost a month.
"Matthew needs to take medication regularly and it is believed he has not taken any with him.
"His family are also concerned as he may become easily confused.
"Matthew may have a black mountain bike with him.
"I am appealing for anyone with any information about Matthew's whereabouts to get in touch as soon as possible."
